In this paper, we outline the foundations of the time invariant, non-unitary covering of quantum chemistry known as hadronic chemistry, we illustrate its validity by reviewing the exact representations of the binding energies of the Hydrogen and water molecules, and present new advances.

The COVID-19 pandemic has resulted in more than 524 million cases and 6 million deaths worldwide. Various drug interventions targeting multiple stages of COVID-19 pathogenesis can significantly reduce infection-related mortality. The current within-host mathematical modeling study addresses the optimal drug regimen and efficacy of combination therapies in the treatment of COVID-19. The drugs/interventions considered include Arbidol, Remdesivir, Interferon and Lopinavir/ritonavir. It is concluded that these drugs, when administered singly or in combination, reduce the number of infected cells and viral load. (...) Four scenarios dealing with the administration of a single drug, two drugs, three drugs and all four are discussed. In all these scenarios, the optimal drug regimen is proposed based on two methods. In the first method, these medical interventions are modeled as control interventions and a corresponding objective function and optimal control problem are formulated. In this framework, the optimal drug regimen is derived. Later, using the comparative effectiveness method, the optimal drug regimen is derived based on the basic reproduction number and viral load. The average number of infected cells and viral load decreased the most when all four drugs were used together. On the other hand, the average number of susceptible cells decreased the most when Arbidol was administered alone. The basic reproduction number and viral load decreased the most when all four interventions were used together, confirming the previously obtained finding of the optimal control problem. The results of this study can help physicians make decisions about the treatment of the life-threatening COVID-19 infection. (shrink)

Rationale, aims and objectives To compare the pharmacotherapeutic approaches to diabetic hypertension of family physicians (FPs) and general practitioners (GPs). Methods A retrospective prescription-based study was conducted in 15 out of a total of 20 health centres, involving 115 primary care physicians – 77 FPs and 38 GPs, representing 74% of the primary care physicians of Bahrain. Prescriptions were collected during May and June 2000 to comprise a study population of 1266 diabetic-hypertensive patients. Results As monotherapy, angiotensin-converting enzyme (ACE) inhibitors (...) (37.9%) and β-blockers (38.3%) were the most commonly prescribed classes of antihypertensives by FPs and GPs, respectively. Calcium channel blockers (CCBs) were ranked third by both categories of physicians. For two-drug combinations, a β-blocker and an ACE inhibitor was the combination of choice for both physician categories. Patients managed by the FPs were more likely to receive a β-blocker–CCB combination (17.4 vs. 14.9%) or a diuretic–ACE inhibitor combination (16.7 vs. 11.4%) and less likely to receive a β-blocker–diuretic combination (11.8 vs. 16.7%) than those managed by the GPs. The proportion of patients receiving antihypertensive combinations was 40.6 and 38.5% for FPs and GPs, respectively. While the GPs prescribed CCB as a monotherapy to the elderly most often, the FPs choice was a β-blocker. Diuretics were less preferred by both FPs and GPs. β-Blocker–ACE inhibitor was again the most preferred combination of both FPs and GPs. FPs prescribed CCB–β-blocker combinations more often than GPs (P = 0.01), whereas CCB–ACE inhibitor combinations were less preferred (P = 0.09). A trend towards excessive use of short-acting nifedipine as monotherapy for elderly patients, both by FPs and by GPs, was noticed. Glibenclamide, alone or in combination with metformin, was the foremost antidiabetic drug prescribed by FPs and GPs. Middle-aged (45–64 years) patients seen by GPs were more likely to receive glibenclamide than those treated by FPs (P = 0.001) and less likely to receive gliclazide (P = 0.01). Combinations of a β-blocker with either glibenclamide or insulin were prescribed considerably more often by GPs.Conclusions Within the same practice setting, a substantial difference was observed between FPs and GPs in terms of preference for different classes of drugs in the management of diabetic hypertension. The compliance of both FPs and GPs was suboptimal; overall, the compliance of the FPs was closer to the recommended guidelines, however. Educational programmes should specifically address these inadequacies in order to improve the quality of health care. (shrink)

A considerable section of the population in India accesses the services of individual private medical practitioners (PMPs) for primary level care. In rural areas, these providers include MBBS doctors, practitioners of alternative systems of medicine, herbalists, indigenous and folk practitioners, compounders and others. This paper describes the profile, knowledge and some practices of the rural doctor in India and then discusses the reasons for lack of equity in health care access in rural areas and possible solutions to the problem.

Pancreatitis is a complex, progressively destructive inflammatory disorder. Alcohol was long thought to be the primary causative agent, but genetic contributions have been of interest since the discovery that rare PRSS1, CFTR and SPINK1 variants were associated with pancreatitis risk. We now report two associations at genome-wide significance identified and replicated at PRSS1-PRSS2 and X-linked CLDN2 through a two-stage genome-wide study. The PRSS1 variant likely affects disease susceptibility by altering expression of the primary trypsinogen gene. The CLDN2 risk allele is (...) associated with atypical localization of claudin-2 in pancreatic acinar cells. The homozygous CLDN2 genotype confers the greatest risk, and its alleles interact with alcohol consumption to amplify risk. This paper is a critical discussion of A.N. Prior’s contribution to the modern understanding of indeterminism and human freedom of choice. Prior suggested that these ideas should be conceived in terms of his tense logic. It can be demonstrated that his approach provides an attractive formalization that makes it possible to discuss indeterminism and human freedom of choice in a very precise manner and in a broader metaphysical context. It is also argued that Prior’s development of this approach was closely (...) linked to his very personal struggles with fundamental religious and metaphysical questions. In his opinion, holding the doctrine of divine foreknowledge together with the doctrine of human freewill gives rise to difficult logical and philosophical problems. It appears that Prior, rather early on, decided to support what was later known as the Peircean solution, although he also analysed what he considered to be the most important alternative solution—the Ockhamist solution. This paper offers a discussion of some of Prior’s published books and papers as well as some of the papers in his Nachlass. (shrink)

Inspired by Williamson’s knowledge-first epistemology, I propose a position on practical knowledge that can be called the ‘know-how-first view’; yet whereas Williamson is one of the pioneers of the new intellectualism about know-how, I employ the know-how-first view to argue against intellectualism and instead develop a know-how-first version of anti-intellectualism. Williamson argues that propositional knowledge is a sui generis unanalyzable mental state that comes first in the epistemic realm; in parallel, I propose that know-how is a sui generis unanalyzable power (...) that comes first in the practical realm. To motivate this suggestion, I put forward two arguments: drawing on dispositionalist ideas, I argue that the practical component of know-how is unanalyzable; based on an investigation of the natures of intentionality and intelligence, I argue that know-how is prior to intentional and intelligent abilities in the order of explanation of agential action. Deploying this know-how-first anti-intellectualism, I then set out know-how-first solutions to two challenging problems for anti-intellectualism: the sufficiency problem and the necessary condition problem. Theoretical and empirical research in bioethics frequently focuses on ethical dilemmas or problems. This paper draws on anthropological and phenomenological sources to develop an alternative framework for bioethical enquiry that allows examination of a broader range of how the moral is experienced in the everyday lives of individuals and groups. Our account of moral experience is subjective and hermeneutic. We define moral experience as “Encompassing a person's sense that values that he or she deem important are being realised or thwarted (...) in everyday life. This includes a person's interpretations of a lived encounter, or a set of lived encounters, that fall on spectrums of right-wrong, good-bad or just-unjust”. In our conceptualisation, moral experience is not limited to situations that are heavily freighted with ethically-troubling ramifications or are sources of debate and disagreement. Important aspects of moral experience are played out in mundane and everyday settings. Moral experience provides a research framework, the scope of which extends beyond the evaluation of ethical dilemmas, processes of moral justification and decision-making, and moral distress. This broad research focus is consistent with views expressed by commentators within and beyond bioethics who have called for deeper and more sustained attention in bioethics scholarship to a wider set of concerns, experiences and issues that better captures what is ethically at stake for individuals and communities. In this paper we present our conceptualisation of moral experience, articulate its epistemological and ontological foundations and discuss opportunities for empirical bioethics research using this framework. This volume is the first book-length study on post-publication responses to academic plagiarism in humanities disciplines. It demonstrates that the correction of the scholarly literature for plagiarism is not a task for editors and publishers alone; each member of the research community has an indispensable role in maintaining the integrity of the published literature in the aftermath of plagiarism. If untreated, academic plagiarism damages the integrity of the scholarly record, corrupts the surrounding academic enterprise, and creates inefficiencies across all levels (...) of knowledge production. By providing case studies from the field of philosophy and related disciplines, the volume exhibits that current post-publication responses to academic plagiarism are insufficient. It catalogues how humanities disciplines fall short in comparison with the natural and biomedical sciences for ensuring the integrity of the body of published research. This volume provides clarity about how to conceptualize the scholarly record, surveys the traditional methods for correcting it, and argues for new interventions to improve the reliability of the body of published research. The book is valuable not only to those in the field of philosophy and other humanities disciplines, but also to those interested in research ethics, meta-science, and the sociology of research. In addition to the aim of mapping and sequencing one human's genome, the Human Genome Project also intends to characterise the genetic diversity of the world's peoples. The Human Genome Diversity Project raises political, economic and ethical issues. These intersect clearly when the genomes under study are those of indigenous peoples who are already subject to serious economic, legal and/or social disadvantage and discrimination. The fact that some individuals associated with the project have made dismissive comments about indigenous peoples has (...) confused rather than illuminated the deeper issues involved, as well as causing much antagonism among indigenous peoples. There are more serious ethical issues raised by the project for all geneticists, including those who are sympathetic to the problems of indigenous peoples. With particular attention to the history and attitudes of Australian indigenous peoples, we argue that the Human Genome Diversity Project can only proceed if those who further its objectives simultaneously: respect the cultural beliefs of indigenous peoples; publicly support the efforts of indigenous peoples to achieve respect and equality; express respect by a rigorous understanding of the meaning of equitable negotiation of consent, and ensure that both immediate and long term economic benefits from the research flow back to the groups taking part. The ontological argument appears in a multiplicity of forms. Over the past ten or twelve years, however, the philosophical community seems to have been concerned principally with those versions of the proof which claim that God is a necessary being. In contemporary literature, Professors Malcolm and Hartshorne have been the chief advocates of this view, both men holding that God must be conceived as a necessary being and that, as a result, his existence is able to be demonstrated a priori (...) . This claim has not gone unchallenged; indeed, numerous writers have argued that neither Malcolm nor Hartshorne has exercised due care in his use of ‘necessary’. That is, critics charge that the arguments of both men have only the appearance of validity, for in their reasonings the defenders of the a priori proof have tacitly assumed that God is a logically necessary being. Whether or not a being can be logically necessary, however, is a quaestio disputata . In fact, until recently the question was not in dispute at all—virtually all ‘competent judges’ agreed that only propositions could be spoken of as logically necessary, and thus that God must be defined as a physically or factually necessary being. But is the statement, ‘a physically necessary being exists’, logically true? Critics of the ontological argument think not; and in support of this view they offer analyses of ‘physical necessity’ which, they feel, not only give meaning to the phrase, but also show that a physically necessary being's existence can be proven only by some kind of a posteriori investigation. Recent work in the Philosophy of Conspiracy Theories has presented challenges to Particularism simpliciter (or Naive Particularism). Critical care nurses are key providers in a high acuity environment. This qualitative research study explored ethical decision making in a critical care practice setting. Fifteen critical care nurses with varying experience and education levels were purposively sampled to assure the representativeness of the data. The theoretical concepts of experiential learning, perspective transformation, reflection-in-action and principle-based ethics were used as a framework for eliciting information from the participants. A new model of focused reflection in ethical decision making was developed. Findings (...) showed that having a role model or mentor to help guide the ethical decision-making process was critical for focused ethical discourse and the decision making. (shrink)

A (normal) system of propositional modal logic is said to be complete iff it is characterized by a class of (Kripke) frames. When we move to modal predicate logic the question of completeness can again be raised. It is not hard to prove that if a predicate modal logic is complete then it is characterized by the class of all frames for the propositional logic on which it is based. Nor is it hard to prove that if a propositional modal (...) logic is incomplete then so is the predicate logic based on it. But the interesting question is whether a complete propositional modal logic can have an incomplete extension. In 1967 Kripke announced the incompleteness of a predicate extension of S4. The purpose of the present article is to present several such systems. In the first group it is the systems with the Barcan Formula which are incomplete, while those without are complete. In the second group it is those without the Barcan formula which are incomplete, while those with the Barcan Formula are complete. But all these are based on propositional systems which are characterized by frames satisfying in each case a single first-order sentence. (shrink)

The argument in theProtagoraswhich starts with an analysis of giving in to pleasure in terms of ignorance, and leads into a demonstration that courage is knowledge, is certainly one of the most brilliant in Plato and equally certainly one of the trickiest. My discussion deals mainly with three problems: Precisely what absurdity is detected in the popular account of moral weakness, and where is it located in the text? On the basis of largely formal considerations I believe that the absurdity (...) is a much less subtle affair than has been thought, and that it is located at 355c-d. What is the connection between knowledge and belief in 358b-d, and how do these two concepts figure in the arguments which precede and follow this passage? Here I want to comment on differences between theProtagorasand other Socratic arguments. How good is the argument at 356–357e which analyses moral weakness as ignorance? There are several problems here, including a fundamental puzzle of the dialogue, namely that, apparently, Plato uses a non-Socratic hedonism to establish the Socratic thesis that virtue is knowledge. Here I agree with those critics who see irony at the expense of the sophists—sophistic teaching is shown to be grounded in hedonism—but in addition I suggest that Plato also offers an oblique criticism of hedonism itself in the course of which basic points of real substance are made. This interpretation fits in with various other features of the dialogue, including the curious fact that, though the main argument proceeds on a thorough-going hedonistic basis, Socrates is subtly but distinctly not committed to that basis. Hence the unity of the dialogue is supported, and as well an attitude to hedonism is given which is more consistent with that of other dialogues, while we are able to see the Socratic paradox in its regular role as a tool of ethical exploration. In his paper, The logic of obligation and the obligations of the logician, A.N. Prior considers Hintikka's theorem, according to which a statement cannot be both impossible and permissible. This theorem has been seen as problematic for the very idea of a logic of obligation. However, Prior rejects the view that the logic of obligation cannot be formalised. He sees this resistance against such a view as an important part of what could be called the obligation of the logician. Prior (...) argues that Hintikka's theorem should not be seen as something paradoxical. On the contrary, it should be seen as a fully acceptable consequence of a basic and reasonable assumption in deontic logic, namely Hintikka's rule. (shrink)

Individuals discovered to have engaged in serial plagiarism in philosophy are few, but the academic publishers falling victim to them are many. Some of the most respected publishing houses in philosophy have recently dealt with the problem of having published plagiarized material. The various responses by these publishers to an instance of serial plagiarism, one that involves forty-three articles and book chapters, provides a real-time snapshot of the practices for correcting the scholarly record. The analysis offered in this article yields (...) a twofold conclusion: first, there is relatively little uniformity among publishers in philosophy for responding to plagiarism; and second, in comparison with the natural sciences the discipline of philosophy often falls short of the accepted practices for correcting the scholarly record. The article considers only public, documented cases of academic plagiarism in philosophy and makes no new allegations of plagiarism. (shrink)

Basham and Dentith argue that the danger of condemning both conspiracy theorists and their conspiracy theories in a democracy has grave consequences. They argue that we should encourage research into public concerns about influential institutions, especially in cases where a conspiracy has been alleged. Rather than dismiss conspiracy theorising, we should, encourage the politically crucial, historically proven gift of watchfulness in the citizen, and its sometimes necessary, proper and correct expression, conspiracy theory.

R. K. Elliott once commended R. S. Peters' work in philosophy of education for being an authentic expression of the self. Many philosophers, probably including Peters, might see this more as a weakness. In an attempt to resolve this difference various kinds of continuity between philosopher and philosophy are explored. These point to an ideal of a two-way, and ultimately ‘organic’, relationship whereby the philosophy expresses the self and the self is formed by the philosophy. Ways of teaching to favour (...) the development of such authenticity in novice philosophers are discussed. (shrink)

Originally published in 1988, M.C. Dillon's classic study of Merleau-Ponty is now available in a revised second edition containing a new preface and a new chapter on "Truth in Art." Dillon's thesis is that Merleau-Ponty has developed the first genuine alternative to ontological dualism seen in Western philosophy. From his early work on the philosophical significance of the human body to his later ontology of flesh, Merleau-Ponty shows that the perennial problems growing out of dualistic conceptions of mind and body, (...) subject and object, immanence and transcendence can be resolved within the framework of a new way of thinking based on the exemplar of the worldly embodiment of thought. The second highest level of the divided line in Plato’s Republic (510b-511a) appears to be about the entities of mathematics—entities such as particular (though non-physical) triangles. It differs from the highest level in two respects. It involves reasoning from hypotheses, and it uses visible images. This article defends the traditional view that the passage is indeed about these mathematical ‘intermediates’; and tries to show how the apparently different features of the second level are related, by focussing on Plato’s need to (...) give an account of how we can speak of many particulars of the same kind, without assuming that they are imperfect copies, in the way sensible things can be imperfect copies, of forms. In his famous letter to A. N. Prior dated 3 September 1958, Saul Kripke suggested the use of branching time in temporal logic. In this paper, however, it is argued that Prior worked with an idea close to the notion of branching time (‘the big Y’) already the year before he received Kripke’s letter. It is likely that Prior’s findings based on this early study can explain why Prior so quickly accepted the idea of branching time when he received Kripke’s (...) letter. (shrink)
